Resource Navigator

The Resource Navigator is a full-time position responsible for coordinating and managing supportive services and resource navigation for guests of Community Care Fellowship (CCF). This position primarily serves within our Mobile Housing Navigation Center at multiple locations and reports directly to the Program Director.

The Resource Navigator should demonstrate a strong alignment with Community Care Fellowship’s mission and core values, working collaboratively with the Housing Navigator to assess needs, determine urgency, and connect individuals experiencing homelessness to essential resources. The goal is to facilitate placement into short-term housing at the Mobile Housing Navigation Center and transition guests into permanent housing within 90–120 days.

Guest Services

  • Connect guests with external agencies and community resources.
  • Prepare Homeless Verification Letters as needed.
  • Provide guidance, referrals, and assistance to address guests’ expressed needs.

Records, Meetings, and Administration

  • Maintain and report data required by grant funding requirements.
  • Enter and update guest records in HMIS and CCF’s Salesforce database.
  • Assist with general staff tasks, including contributions to social media content, fundraising support, volunteer engagement, and other organizational activities.
  • Attend Board meetings and Nashville-Davidson County Continuum of Care meetings as needed.

Resource Navigation

  • Network creatively to identify and develop new resource opportunities for guests.
  • Coordinate services and advocate on behalf of guests with partner agencies and service providers.
  • Build relationships with individuals experiencing homelessness and provide them with resources, referrals, and emotional support.
  • Help guests navigate complex social service systems to obtain essential needs such as identification, healthcare, food assistance, transportation, and housing.
  • Collaborate with the Housing Navigator to support transitions from encampments to permanent housing through the Coordinated Entry System (HMIS).

Program Coordination and Support

  • Work with CCF Resource Navigators and staff to coordinate the Mobile Housing Navigation Center – Atrium Flat and Madison Christian, including tenant referrals from the Office of Homeless Services.
  • Partner with the Program Director and other team members to identify, enroll, and support guests through housing and resource navigation.
  • Support the Employment & Education Manager by scheduling counseling appointments, assisting with paperwork, and helping guests find long-term, sustainable employment that promotes self-sufficiency.

Training and Professional Development

  • Complete all required and recommended trainings for professional growth, including:
    • Housing Navigator Training (Office of Homeless Services) – required
    • HMIS Coordinated Entry Training (Office of Homeless Services) – required
