The August 31 deadline to complete the Health Check and Blueprint for Wellness (BFW) is quickly approaching.  If you have not already completed either task, please don’t put it off any longer.  

HealthFlex participants (and spouses, if covered) both need to complete the Health Check to avoid a higher deductible in 2026. Participants and covered spouses can also each earn $100 and up to 140 Wellness Credits for completing the Blueprint for Wellness (BFW) by the August 31 deadline.

The Health Check is Quick—Easy—Confidential and can be completed in 15 minutes or less. If you did the Blueprint for Wellness® biometric screening, your health data is uploaded automatically into your Health Check—for faster, more accurate data entry. Health Check is confidential! Your church, annual conference, employer, and Wespath cannot see your Health Check results and other health information.

Health Check is administered by Personify Health.  You can use your computer, tablet or smart phone, use the Personify Health app or visit login.personifyhealth.com and go to the Health tab.

The Blueprint for Wellness (BFW) is a health screening that includes a blood draw and a few measurements such as blood pressure and waist circumference.  If you complete the BFW, you earn $100 in Wellness Credits through Personify Health.

You can complete at a Quest Diagnostics lab at your primary care provider and then submit a physician form or you can use the Self-Collection kit.  You will need to register by phone at 855-623-9355 (Employer Group: Wespath2025) or log in to your Personify Health account and find Blueprint for Wellness under the “Benefits” tab or participant can go through Benefits Access.
